I think it depends, the tag seems like it would fit stories where the character is either 1. confirmed transgender by creators or 2. contains language referring to the character as trans and/or has transgender imagery The term also is gender agnostic, so merging it with shemale isn't quite right either, I could see the tag being renamed to "transgender" to get rid of some ambiguity, given a better description, and being actually applied to works.
