Most of the time this tag kinda refers more to Japanese colloquial phimosis rather than actual medical phimosis.
Everyone is born with their foreskin fused to the glans of their penis, maybe a biological adaptation to protect the glans from injury or irritation when you're a baby. Generally by the time you hit puberty at the latest, the tissue holding the foreskin to the glans dissolves and the foreskin becomes retractable, unless the opening at the end of the foreskin is too tight and won't retract or is difficult/painful to do - which is what true phimosis is.
In Japan for whatever reason there's a cultural insecurity that a REAL man always has his glans exposed. If you're seen with your foreskin unretracted you might get teased for having a baby penis, and people kind of informally refer to a foreskin that isn't retracted (or goes back to unretracted on its own when you're not hard) as phimosis even if it retracts just fine.
